I think you have two things in one:

I don't agree. You want to take the assessment even they haven't understood the requirements? I would say, No. The expected answer is: Yes. Same with the other three.
So, the quiz is the correct tool, students need to pass with 100% marks. If not, for example, go back and read the requirements and come back, etc. You can allow unlimited attempts until quiz passed with 100% score.
B.

This is an entirely different thing. If the answer is Yes, you have to take special precautions. Something like the Choice tool suits this better. You have to set e-mail notifications. Only thing, I don't know how to set Moodle e-mail only if the answer is Yes.
