This is a result of the encrypted file system in Moodle 2. There are all sorts of solutions, e.g.
- https://moodle.org/plugins/view/block_repo_filemanager
- https://moodle.org/plugins/view/block_coursefiles
- https://moodle.org/plugins/view/block_my_external_privatefiles
- recently seen https://moodle.org/mod/forum/discuss.php?d=323727&parent=1299968
I haven't used any of them!

1) I understand that you are looking for something like the https://docs.moodle.org/en/File_system_repository to which you can upload files straight, i.e. without the going through the Moodle web-interface but need to be private to somebody. No, I don't think such a thing exist, at least not until Moodle 2.7.
2.a) https://docs.moodle.org/en/Google_Drive_repository
2.b) https://docs.moodle.org/en/Dropbox_repository
It is worth studying the https://docs.moodle.org/en/Repositories docs.

That is the problem with intermittent problems. When they happen either you are not aware or you can not enter the shell!

The standard solution is to run a performance monitor which continuously records a set of system parameters. See "How do you monitor your Moodle server?" https://moodle.org/mod/forum/discuss.php?d=192162.

http://www.webalizer.org/ is the classic, might still look web1-like, but easy to install.
