That is the prescribed method:
- Delete all the questions of that type.
- Then Moodle should offer to Uninstall the question type on the admin GUI.
- Only then remove the code directory.

Could you explain your procedure in detail?
 
I understand what you say this way: To solve the problem of duplicated questions move to a fresh Moodle instance by Course backup in the old instance and Course restore in the new instance (not through Moodle migration). During that process set the options not to import question banks.

Then? What'll happen to the existing quizzes and their attempts? How do people reuse the old questions? How do they migrate the old questions to the new instance?

I agree with you in all the points except for the conclusion:

You have put two things together which need individual attention:
1. Quizzes using only the core question types.
2. Additional question plug-ins and code customizations

I agree that people who do 2) should know what they are doing. So a general warning like "you might encounter problems" is sufficient. But in 1) you find all sorts, including non-techies and the clueless. Developments in Moodle should not leave them in the cold.
