Unfortunately, the export/import of images in Glossary definitions does not work. See MDL-28526. You may add your vote to the 29 votes in that issue, but there is little hope that this will be fixed any time soon.
I have no idea how the workaround mentioned in the documentation here can work.

Thanks for mentioning this. I have reduced the text of the README file on the master branch, which is actually empty to explain that you must switch to a current STABLE branch to download Questionnaire.
