Hi David,
In my opinion, the H5P Questionnaire activity is only a "game", it's totally useless as a proper survey tool. Of course, having maintained the Moodle Questionnaire plugin for a few years I may be biassed , but since you are using Moodle I can only advise you to use the Questionnaire plugin. It's not part of Moodle but easy to install (if you are the admin of your Moodle site or if you can ask your admin to do it for you).

As far as I know Moodle Cloud does not have the full Moodle features that you have on a "standard" Moodle site. I don't use it so cannot help, and leave it to others to answer.
