Hi Pete,

Your problem seems related to your recent upgrade of the Questionnaire module. What happens is that when you try to create a new instance of Questionnaire, the Questionnaire module is unable to update your moodle database. Something must be wrong with that database, some fields may be missing.

Can you tell us more about the story.

  1. Were you previously using a version 1.8 of Moodle (and of Questionnaire)?
  2. Did you a) upgrade from Moodle 1.8 to Moodle 1.9 and then upgrade from Questionnaire for moodle 1.8 to Questionnaire for moodle 1.9?
  3. After the Questionnaire for moodle 1.9 upgrade, did you, logged in as Admin, go to the front page of your moodle site and click on the Notifications link to update your moodle database?
  4. If not, can you do it now and see what happens?
  5. When you create a new questionnaire, instead of Create new, can you try Copy existing (supposing you have existing formerly created Questionnaires in that course)? Does it work?
  6. Which type of moodle database are you using (mySql, MS-SQL or other?)
  7. If you had already created some questionnaires in your moodle site, do they still work?

Hi Gary,
Please be aware that in Moodle the word "lesson" refers to a specific activity, the Lesson activity. In your message, are you using the word "lesson" in a general meaning, or are you referring to that Moodle specific activity? When you say "students say they have completed the entire course" what does that mean exactly? A little further down you say "the issue appears to be whether the system sees the student as having completed' the lesson". For you, are "completing a course" and "completing a lesson" the same? Please re-state your case using the Moodle terminlogy, to avoid ambiguity and so we can help you.

You are right, there is no "attempt" capability for the Lesson capability (in Moodle 1.9). I guess that the reason is that the Lesson module is no longer actively maintained and was never updated for capabilities, unlike the Quiz module. I have created bug report MDL-20087 for which anyone interested can vote.
