There's no 'context' for plugins. You can allow roles to be added in a number of places (e.g. site, course category, courses and so on) but 'plugin' isn't one of them. Where you want to do this probably depends on what sort of plugin we are talking about.

However, Manager roles are generally at a higher level - site and course category. They don't make much sense elsewhere.

Uncommenting in php.ini only works if you are running on Windows. Are you?

I'm assuming that these are HTML emails (or trying to be). I would look carefully at the raw source of the email in your email client. Try to spot where it's going wrong such that the HTML isn't being rendered properly.

Have you changed any language strings or installed any plugins that might be related (or at all)?

You need to look at your server error logs.
