It depends what you mean by "custom design". Do you just mean the "look" (or the Theme as we call it) or something else?

If you're really asking... "how do I know my students have watched the video?"... have you considered a quiz to test their knowledge?

I can find no record of a course format format_topics_advanced in the plugin database (or anywhere else). So where did that additional plugin come from?

Anyway (again), I would assume that selecting that course format is why it looks like it does.

Might depend on the rest of that table. Are they queued for deletion, for example?

MySQL 5.7 was "end of life" nearly a year ago. This is definitely not a good position to be in.
