Easy things first - have you tried setting $CFG->reverseproxy = false; ?
I've set up Moodle behind reverse proxies a few times and never needed to set that.

No. It means that completions will be cleared and then immediately recalculated for all users. Users who completed before will be shown as completed again. Others will not. The issue that you might care about is that the completion timestamp will be bumped to "now". With that caveat, it's generally a safe thing to do.
