If they are still having the problem, they might be experiencing the issue mentioned here
If not, there are some new changes that were recently updated in Moodle 1.9+ and should be available in 1.9.1. I have had clients describe similar inconsistencies and I'm hoping that the changes that were made help to resolve those issues.
