BUG: The course editing menu (kebab) is not displayed after hiding or showing. 4.1.x

I have version 4.1.9 and this see this error.

STEPS TO REPLICATE

1. Enter any course

2. Active edit mode

3. Create a new activity or resource.

4. Select Label.

5. Add any text to the label.

6. Save and return to the course.

7. Click on the menu (kebab).

8. Select hide/show.

That doesn't look like standard Boost or Classic styles. What theme and version or any custom code / CSS etc. Any other non-core plugins installed?

The theme is: Boost only change the color.

We have the version 4.1.9.

format_onetopic

mod_questionnaire
block_panopto
block_quickmail
