When I download version 1.5.X of the questionnaire module and unzip the file there is only one file inside the questionnaire folder: locallib.php. This throws a "Module "questionnaire" is not readable - check permissions" error in red at the top of the Moodle page when I add it to my mod folder on the server. I am hoping the 'Latest' download (which contains all the files) works with 1.5.3+. Just thought I'd let you know so the 1.5.X version can be updated.

I had the same issue and just went to the Modules and Plugins and was able to get a good copy there.
