I installed and this is what I see on the top of my site at http://www.globalyeshiva.net/etorah/

I also see:

"questionnaire" is not readable - check permissions

and

Module questionnaire: /home/content/y/e/s/yeshivanet/html/etorah/mod/questionnaire/version.php was not readable

The only file that exists in that folder is locallib.php ther is no version.php

version.php was also not downloded with the files to install

I just compared my moodle from Sunday last week with this weeks version... and...

/mod/questionnaire
/mod/questionnaire/locallib.php

Are new additions...

I do not know what the solution is... but I am assuming lots of people will be getting this... if they update this week.
