The forum completion conditions are all met but the forum activity does not show as complete in the activity completion report.
Each student has made 2 discussions and 1 reply and they have all received whole forum pass grades which show up in the gradebook.

Hello,
As mentioned in the Activity completion FAQ, please start by checking in Site administration > Server > Scheduled tasks that the task Calculate completion data is running frequently.
Otherwise, I suggest you try unlocking the completion settings and un-ticking 'Require view' and 'Require posts' (since if they have created 2 discussions and posted 1 reply then they have viewed the forum and made 3 posts) then save changes.

"Calculate regular completion data" runs every minute. (I double-checked.)
I unticked 'Require view' and 'Require posts' and saved the changes.

Sorry to hear that the problem persists, though it's good that you're narrowing down the possible causes. How about seeing whether unsetting the require grade condition does anything?
Otherwise, I suggest you check whether the problem can be reproduced on https://sandbox.moodledemo.net/ and if so, create an issue for it in the Moodle Tracker.

No worries. That's what problems do. They persist. That's their raison d'être! 
Well, I did a variation of what you had suggested and unticked "discussion" and "reply" and just kept "require grade", and, lo and behold, everything worked like a charm.
Two takeaways from this:
1. To me this looks very much like a bug.
2. There should really be a "passing grade" option available for forum completion and not just only "grade".

Good work tracking down the cause of the problem. 👏 It does indeed seem like a bug. If you have chance, please create an issue for it.
Re. a passing grade option, I think it's only implemented for Quiz and Assignment so far. Maybe you could create an issue for that too and encourage people to vote for it? (I checked and couldn't find existing issues for either the bug or a passing grade option.)

Well, I did try to test it in the sandbox.
As admin, I created a course and within it a forum, and allowed self-enrollment. I then logged in as a student on a different browser and made a discussion as a post as required. I then graded it but then suddenly everything went haywire. The course I had created vanished from the sandbox (and most probably the face of the earth too).
And I'm afraid I haven't got the time at the moment to do it again.
The good news is that the second problem is already dealt with in Moodle 4.

It sounds like you encountered the hourly reset on the sandbox demo site. Sorry I should have mentioned to try reproducing the problem just after the hourly reset to give yourself plenty of time.
Thanks for sharing the good news about the passing grade option for forums in Moodle 4.0 onwards.
