Hiya

We host our Moodle site externally and have been advised by the host company to post about our issue here as there appears to be no resolution. 

We are having a sitewide issue where teachers are trying to edit their courses but when you click to add an activity or upload a file, it appears to be deadlinks, ie the page does nothing. We find if you fully refresh the web browser it does solve the issue temporarily but you have to do this each time you wish to make any edits. Unforutanely this is not viable to keep doing or expecting staff to do. 

We are running Moodle 3.9 and it is the latest version of that, the host has advised not to upgrade to 4.0 and to wait for 4.1. 

They fully reset the site yesterday hoping this would fix the issue ( it didn't), debugging was turned on and checked and nothing. 

We are really stumped as everything else is working absolutely fine, I would appreciate any suggestions as we are very keen to resolve this issue as soon as possible.

You've given description of the issue without any technical information cept to say nothing reported in debug.  That's makes responses difficult - more like guesses ... and predict will beget more questions about setup so i'll begin that ...

We know version 3.9 ... but how about something more exact ... the point release ... like 3.9.xx.   Along with DB service and version, PHP version, etc.

Operating system of server is?

What 3rd party plugins running?   How about theme?

Do you have access to server error logs ... nothing there?

Think I would agree with provider for this one point ... have this issue now, upgrading may not be the answer - could be jumping out of the pan and into the fire so to speak.

In another of your postings, you had an issue:

Cannot create local file pool directories

Was that resolved?

We are using Hubken Group Super theme
3rd Party plugins include: TurnitIn, Clickview video, Custom Certificate, GO1, Hotpot Module, H5p, Notetaker, PDF annotation, Subcourse, Coderunner, Jmol, Chemistry Editor, Grid Course format, Edwiser course formats
I do not have access to server error logs but I have been reassured they are clear.

That issue was resolved - I believe the host found the fix for that one!

Thanks for update on techie info.   Info related to Linux is strange to me.   So deeper dive:  

Is your server hosted in a VM? What's the VM software?   if that environment, is your server the only Guest OS on that physical machine?

Is it a standalone?   A single server with web service, DB server, and storage of moodledata on the same server?

Does it have a WAF - web application firewall?   If so which/what?  A WAF might produce behavior you've described .. they tend not to return accurate error info ... or none at all ... to avoid detection.

The WAF could be enforcing a rule that's really a false positive ... an authenticated user, logged on, and editing something triggers a rule and you get nothing/nada.  mod_security has been known to act like that.   So the log to look at is the audit.log for the WAF.

Although you said a refresh of screen 'clears' the issue ... which leads to the next question ... caching.

System running a cache service?

Think you could see that and it's settings in PHP info link Server Admin Server.

Did see the following in viewing site front page with FF Tools ...
