Look at other MS Word template for making Moodle questionnaires. It produces XML output.

I've used it... en masse. I had my students using it... found lots of bugs... the idea is great but the whole thing should, IMHO, be written in javascript(more people know js than MS or OO macros) so that it can be more portable (any browser) and so we can get more eyeballs into the code and get something working ... (more to do over my spring break?)
