While I still don't see this as a full solution, I did realise that a Cloze with a MCH style question can simulate the horizontal style survey questions:
Then using the embed question report, you can get the responses.
You don't seem to be able to download the response data for the users, so this is a very manual process, but might be suitable for small cohorts.
