I am developing an activity module but it does not show me the icon ,
I have icon.png and icon.svg under mod / evaluation / pix but no icon next to the module
is there a code that must be written to display me the icon ?

as far as I know the only thing required to display an icon is to have a file named 'icon.png' or 'icon.svg' in the pix folder, so no extra code has to be written.
have you tried purging the caches? (Site administration -> Development -> Purge caches -> purge all caches).

yes I have purged the caches but it does not show me the icon

Try only keeping one of the files - possibly the .png file and see if that works - also not sure on Windows - but you might also have a permission problem - can the webserver user actually access the folder or file. Try loading the image in your browser directly.
