Dear, I have a question about a scenario to solve.
I need that a teacher can have within the activities to use for the students, something that allows having a checklist only for good or bad in each item and that in the end I get the percentage granted.
This is loaded by the teacher as the student answers a series of questions.
I thought of a survey put together by us but I'm not entirely convinced.
Otherwise I thought of a google form but there I have to exit MOODLE.

Another possibility, since you mention, "having a checklist," is to use the Checklist plugin.
