There is already a mechanism to handle this - you just need to use it!
The reason questions in quizzes don't get accidentally deleted is because quiz implements this method: https://github.com/moodle/moodle/blob/master/mod/quiz/lib.php#L1438. You need to do the same in the lib.php of your plugin.
