Is there any way to get larger background images in these questions? 600x400 is simply not big enough with modern resolution monitors.

Ah, I also see that this is mentioned at the bottom of the docs page for this question type - I completely missed that. The link simply directs to the thread, but the link to the actual post (by Jamie Pratt) is https://moodle.org/mod/forum/discuss.php?d=358512#p1446513.

I am going to ping our Moodle partner and see what they can do.

Interesting workaround though. Unfortunately, I don't think I will have access to the raw HTML as this is a Moodle that is hosted via a partner.
