In scheduler event, when the student choose the slot we want to ask some questions like which department, why he/she is attending this class. Is there any form options possible we can create in scheduler events ?

Thank you for your support. I have examined the Scheduler book property. It gives me idea but we need a form structure. Students should fill the answers of us then enroll the empty slot. Here, students only can upload file or read the descriptions. We need a regular data from students instructions which is collected from students who visit enroll the slots. If we use scheduler we can get collect untidy answers from students and it will be hard to control the data. Where I can check any form and scheduler is in the same speciality? I check the plugins but it 's too much to distinguish.

I'm afraid that Scheduler doesn't have this feature at the moment. The booking form options are exactly those that are described on the page I mentioned, no more and no less.
You might try to combine a Choice or Survey activity (or similar) with a Scheduler, making the Scheduler visible only when the Survey has been completed (see https://docs.moodle.org/37/en/Activity_completion). This is just a suggestion, you'd need to see whether this works for you.
