I hope this is the right place to ask this question. I've downloaded the questionnaire module and unzipped it into my mod directory. Then logged in as admin and gone to the admin page. I understand that installation is supposed to be automatic at this point and I should get a message informing me that it's been installed. However, I get no such message and the module does not show up in the Modules config section.
The rest of moodle (v1.52) seems to be working just fine. I'm guessing there is something not quite right, but I don't know where to begin to look.

It should install, yes. Can you verify that you see a 'questionnaire' subdirectory in your '/mod' directory?

I have this problem also!
The map is there but no action in my adminpage!

i see with explorer all the files of questionnaire they are in c:\moodle\moodel\mod\questionnaire
Starting moodle admin doesn't see questionnaire in modules.

Perhaps you could just check you don't have a questionnaire folder within the questionnaire folder. I've found this can happen when unzipping, and this may account for the module not installing correctly.

If 'moodle' is your home directory, then it looks like you may have one too many subdirectories. Are you sure your 'mod' directory should be below a directory called 'moodel'?

Mine tries to install and i get this message:
Module questionnaire: /var/www/moodle/mod/questionnaire/db/mysql.php was not readable
I would download it again and reinstall. It sounds like that file isn't readable for some reason.

Im getting the very same problem.
its there under C:\moodle\moodle\mod\questionnaire but wont appear when i log on to the site (admin).
any ideas?
