We want to create a global survey that can be filled as many times as needed, at the end of a course. We currently have a template but we want to auto-populate certain information on the survey such as the date, course name, trainer and site (location of the office). Is there a way to do this?

There is nothing in questionnaire that can do that. But you could always use a filter plugin, like 'Filtercodes'. Then you could add the text you want into the content of a question.
