Choice - Check box does not show

We created two courses. Both courses us Choice. One course shows checkboxes for the choice. The other course does not display the checkboxes. We want the checkboxes to be displayed for the second course. We need help to troubleshoot this issue. Refer the attachment for the screenshots.

The checkboxes opposite the choice activities on the course page are activity completion checkboxes. Please see the documentation Activity completion for details of how this feature works.
