Documentation on basic concept of "issuing" a certificate

Hi. I'm not sure if I've missed something somewhere... but I can't find any documentation/explanation on the following basic concepts of this plugin: 
1. How/when is a certificate "issued"? Is it issued when a user accesses/clicks the activity? And/Or when they have completed/achieved a specified goal/task... and if so, is this just set up using the "restrict access" settings within the plugin?
2. Who can a certificate be issued to? EG How do I set it so that only users with a "student" role in a course can be issued a certificate

HI Richard, 

I have also struggled habit with working this out. We issue the certificate by restricting access based on completion of an attendance activity. Generally this works well in any case where you are about apply a restriction. 

I still have not worked out what logic is applied to teachers being able see students certificates but not having one generated for themselves. I have just embarrassed myself by sending out a batch of certificates to teachers by granting them access to them. 

Any helpful answers would be most welcome.
