Another possibility would be to present the statement of responsibility using a Label resource with its completion settings set to allow students to manually mark as complete - then restrict access to the quiz based on the completion condition of the statement. This would give a checkbox to the right of the statement, and checking it would "unlock" the quiz. You statement may need to include something like, "by checking this box, I agree...".
