Hi,
I want to put our Request for Change Form (RFC) into the LMS but have some fields populated by Moodle if they are contained in the profile fields (i.e. teachers name, phone , location etc) and the rest to be completed by the teacher.
I also require that the teachers upload a couple of documents to the attach to the RFC, and it would be fantastic if this could all be done in the same area.
I have looked at Feedback, questionnaire and application plugins, but they either populate the data without allowing for detailed questions or restrictions.
I am using version 3.3. Any ideas would be great?
