We have around 120 students, and most of the time, around 10% of the students' grade were not being recorded after 1st attempt.
The student have same roles, and they are enrolled in the course. We check the logs the the user really attempted the quiz. You can notice here that there's no log like "User graded" here even though they attempted the quiz and reviewed.
When we try to lift the attempts and the user retake the exam, the grades were successfully recorded. Of course, we do not want for users to retake the exam again due to this issue since they already had their first attempt already.
This has been going on for 3 weeks now, and im going nuts how to resolve this.
I hope for your advise. I'm using Moodle 3.1.1
