Question_Answers and Question_Attempts Do not match

Question_Answers and Question_Attempts Do not match

I am using Moodle 2.9 and I encountered the following discrepancy between the Question_Answers table and Question_Attempts table, where:

Question_Answers has the answer choice: Obstructing  Administration

and Question_Attempts has as rightanswer and responsesummary: Obstructing Aministration for the same question. 

There is no answer choice Obstructing Aministration. 

I was wondering if anyone has experienced this issue before, or if anyone has an idea as to how to prevent it from happening.

The data in question_attempts is really just some cached information to make displaying the reports faster. It gets set when the question is attempted.

I expect what happened is that the teacher spotted their typo, and edited the question after some students had attempted it.

You can fix things by re-grading the quiz.
