You run into character encoding issues with Arabic language questions, so text import formats like GIFT may be problematic. Question XML would be the obvious choice, but then you run into the issue of what editor to use.
You could also consider using the WordTable plugin I developed, which allows you write questions in Word and import them directly into Moodle via the Question XML format. It supports right-to-left languages such as Arabic and Hebrew.
