Having played with your XML export part of me thinks that cannot be the reason because I got the error and they were never duplicated on my system. However as it fixed it on your system that sounds like a very plausible answer. I suspect the answer is somewhere in that xml export but I just don't know where. Either way it gave me a good refresher on the OU combined question type which was very interesting.
