Error code: cannotfindpages

Some of our students get the following message, when they click on a lesson

"Could not find lesson pages"

Debug info: Error code: cannotfindpages. 

The error also occurs when a student is busy with a lesson, and tries to navigate to a next page.

Moodle 2.9.1. The site has been in use for about three years now and the problem only recently started.

As I have said; it only happens to some students. 

Can anyone shed some led as to possible reasons for this?

Hello,

I tried a quick search of the Moodle Tracker for 'cannotfindpages' and found MDL-51097. Do you think it might be the cause of your problem?

Thank you for answering. The lessons do not consist of random questions and clusters. They are straightforward, continuing to the next page if the question is answered correctly or redirected back to the video page, when the answer is incorrect. But the error is the same.

I recently upgraded the site to 2.9.4 which is supposedly a fixed version, but the error persists.
