We have been restoring the courses with the option: 'Restore
as manual enrolments' ticked. What does this do?

What it says. Any user enrolments will come back as 'manual' regardless of what they were in the original course. This is useful if you restore on a site that does not have the same plugins enabled. For example, if you used external database enrolment to enrol your students on the original site but you don't use that on the target site. You have the option to convert them to manual enrolments.
