Menu Icon for Questionnaire?

Hi all, I really like the Questionnaire module. I just have one small bug:
When I add a link to one of my Questionnaires to the opening page menu block it doesn't show an icon (like the other modules) but spells out "Questionnaire" which ends up making the other icons shift left. Well here's a screen shot so you can see what I'm saying...(the Questionnaire-- in question-- is called "Chapel Feedback Form")

Because the Questionnaire icon is missing, alt text is displayed instead. Please check the discussion Icon for the Questionnaire module? Depending upon your theme setup, you may need to add the icon to /theme/customtheme/pix/mod/questionnaire/.
