OK, I ultimately answered my own question, namely I could restart and everything would eventually work out.

The source of the problem was ultimately outside of Moodle.  It turns out that our DB had nearly 15,000 (!) connections to it due to a misconfiguration of things totally unrelated to Moodle.  This caused the DB to be super slow (bit suprise).  It hung on the command above because the log table was really large (over 133,000 entries in almost exactly one year of use) and eventually timed out.

We got the DB so that it was responding plausibly again, and then going back into the admin page started the upgrade again.  It took quite a while, but once it waded through it all things seem to work fine now.

It's really nice working with software that handles this sort of goofy situation so gracefully.  Congratulations and many thanks to all the Moodle developers that helped bring us such a robust tool!
