I have deleted every reference to the questionnaire module and
reinstalled it but the errors I was having before are still occurring.
When I install questionnaire and then click on Administration to set
the tables up, I get the following error:
Fatal error: Cannot redeclare questionnaire_upgrade()
(previously declared in
/home/webadmin/childbirthinternational.com/html/study/mod/backup/db/mysql.php:3)
in /home/webadmin/childbirthinternational.com/html/study/mod/questionnaire/db/mysql.php on line 3
Any ideas? This is stopping me from setting up any questionnaires adn I thinkis causing problems in other areas as well.

You need to remove that 'backup' directory from your 'mod' directory.
