It is a standard feature added in Moodle 1.9.0. (MDL-648, fixed in 2007.) Just how old is your Moodle site?!

Is cron running OK on this server? https://docs.moodle.org/19/en/Cron
Do other emails come through? e.g. forum subscription emails.

I concur with Rens when he says "The date and grade would be nice information."
Also, a feature which may allow an email sent to an external email address (for example: A student in enrolled into a course by a 3rd party. An email notifying the 3rd party that the student passed a quiz and the date, grade of the quiz.)
