Is there any way to set a database activity up so that the teacher gets a notification upon a new entry?
I would like to use the database activity as an online forms system for our staff; but would need to be able to notify the administration team when a new form was submitted.

Only available in Moodle2.8. The new feature called: Event Monitor. You can get notifications for pretty much every thing! Yay!
But can the Event monitor be set up so that a course teacher can receive notifications?
If so I think we will need to plan to upgrade to 2.8 ASAP.

As soon as admin sets up notification rules, teachers will see a link to the list in their profile settings navigation menu. Clicking the link will take them to the list where they can subscribe to any of the rules there.
You can also try the contributed Dataform module for this purpose. It is a major enhancement of the Database activity and can be very useful for staff forms as it allows you to limit access to form fields by roles or individuals in various ways. For notifications it allows you (or the activity creator for that matter) to create activity internal notifications and assign them to roles in the activity. You can also include in the notification message entry specific information in a flexible layout and condition the notification by entry content (e.g. send notification on forms with particular content; available from next release for 2.7 and 2.8 this coming weekend). It doesn't have yet subscription function, but it's in the pipe including reminder functionality.
I had looked at the Dataform Module Itamar and wondered if that would be the way to go as opposed to the Database module; but had wanted to try and keep my Moodle install as close to core as possible.
I will bring up a 2.8 install at home and have a play around with the notification options to see if it will do what we need; if not I will look at the Dataform module.
