if you have Moodle 1.4.x, it sounds as if you may not have moved the language files from "mod/hotpot/lang/en" to "lang/en".
If you have Moodle 1.5.x, you will need to enable the HotPot module on the "Administration -> Configuration -> Modules" page.
Sometimes it takes a few clicks on the admin pages before the updating process kicks in. The page that actually detects whether an update is necessary or not is "admin/index.php", so you could also try typing the URL for "admin/index.php" on your Moodle site directly into your browser.
