My apologies, I missed the email notification that you replied. Could I upload the zip file to my Private Files section, and then send it to you that way? I'm not sure if the textbook publisher would want me to post a link to it publicly.
At one point I thought I had figured it out. Running my test Moodle site in debug mode revealed that the HTML code for the "&" sign was causing the import to fail (those signs were in some of the question titles). However removing them did not solve the issue.

Just a bump to say that I am too experiencing this issue with a faculty member. I am also importing question packs from Cengage Learning into Moodle 3.0 and getting the same error message when I load the zip file.
