We are very new to Moodle and we are currently setting up using 2.5, but we are experienced php developers.
We would like to modify or create a course based user answers to a diagnostic quiz.
Is there a plug in to do this or a standard process ?
Similar to the above, can you have section of a course that varies based on a user or a cohort or a group ?

(Actually, you have asked two questions in one post -it is better in future if you ask only one question per post; that makes it easier for people to respond.)
I am not quite sure what you mean by a modifying a course based on user answers to a diagnostic quiz but you can certainly alter the path a user takes according to their previous responses. There is a module which allows for branching - ie, different paths according to how the user replies. See: Lesson Likewise, you can reveal activities selectively according to criteria you specify. See Conditional activities
For your second question, the above still applies and you can also use Groupings to restrict sections to certain users.
