What is the proper way to upgrade this plugin? The last version installed was 20080707006, and then we upgraded our Moodle from 1.9 to 2.5. I'm trying to update the plugin, too, but am having difficulties - going straight from 20080707006 to version 2013100501 didn't work, so if anyone can shed some light on the proper way to go about this, i'd be most grateful

First I don't think it's possible to upgrade your Moodle directly from 1.9 to 2.5.
Second, you must follow the procedure explained on the Questionnaire plugins page:
NOTE: If you are upgrading to Moodle 2.3 or greater, make sure you have upgraded the questionnaire module to the latest version of Moodle 2.2, in your Moodle 2.2 codebase before upgrading to Moodle 2.3 or greater.
