From what I can see on your latest screen print, you are certainly not using the most current version of Questionnaire for moodle 1.9 that is available from the plugins repository.
Try upgrading to this version here: https://moodle.org/plugins/download.php/424/mod_questionnaire_moodle19_2008060405.zip
That's version 1.9.1 (Build - 20120106) (2008060405)
