How should you report errors in passed tests, is it good enough to descripe it in a comment or should you create a new record in the tracker? I ask because testers can not change a passed test to failed.

Hopefully it doesn't happen very often that a test is passed then we later find out that it failed, but if it does, then you can change it it to failed by first resetting it and then clicking the fail button. As usual, the procedure for failed tests as described in the QA testing guide (making sure the problem is reported in the tracker etc) should be followed.
