Add activity or resource Weird Dialogue box

When adding an activity or resource I get a weird dialogue box as attached below.  I am also not able to run slash arguments as the theme pictures wont load.  I have recently upgraded to Moodle 2.3.2+ (Build: 20121018) where the problems appeared.

Any help would be appreciated.

Hi Ashley,

The activity chooser was added in Moodle 2.3 and should look like the screenshot in the documentation on Course homepage.

Is your site using a standard theme?

Hi Helen, Thanks for your reply. I know what it should look like the problem is that it doesn't look like that smile The theme I am using is a standard theme and it does the same thing to each of the standard themes that I change to.

I suspect some code is missing, possibly some CSS. Go to admin and turn on debugging and see if any errors are displayed.
