Edit Commands Disappear in View and Grade Submissions

Greetings - I did a search through the forum and I could not find any postings regarding this little problem.  We have found in several Assignments that when faculty go to View and Grade Submissions - Edit - when they try to move the mouse to click on update grade - the commands disappear.  We are in Moodle 2.3.2+, using Firefox and on a Linux server.  As always any help you can provide is appreaciated.

We have something similar going on in a completely fresh installation of Moodle 2.4.2 with the Cover theme.

When a grader clicks on the + symbol (View full) in an assignment folder to see a student's Online text, instead of showing the full text, the summary disappears and the field is empty (see pics).

Can anyone shed any light on this?
