Questionnaire (plugin)

Having a similar problem to the ones mentioned in this thread and the one where the fixed file is posted. New install, I try to setup a questionnaire and I get a blank screen after pressing "continue." My address bar shows the link to "manage_survey.php" but nothing displays on the screen. Quite frustrating. Any ideas on this? (If someone needs access to debug let me know)
