Attached is a working translation of the version provided here by Daniel Scheidegger. I've updated the toolbar to work in 2010 correctly using the Custom UI editor. The files needed for customisation are included in the package - they aren't needed for the template to work, only if you want to edit or customise the toolbar.
The Template still doesn't generate new documents correctly (will freeze Word), but if you copy and paste the file and then edit it will export XML correctly (and import into Word). I'm having issues with importing images into moodle from the quiz - but I think that has something to do with apache on windows (inserting the drive and moodledata root folder name in the image url). I'd be interested to hear people's success after testing it.
This works with Moodle 1.9 (I believe there are some changes that need to be made for it to work with 2.0+).
