I have been trying all morning to download the version 1.9 module for Questionnaire. I cannot find it anywhere.
I'm looking for the version to run with 2.1.4 and can not find anything either.

We also noticed it has gone missing from old modules and plugins database.

@Tabitha: Yes, it's been moved from the old to the new Plugins repository.

It is still there (http://moodle.org/mod/data/view.php?d=13&rid=84), but it may be waiting for approval due to changes as well. But you may as well get it at the new location.
